Sometimes you only notice: It cannot go on like this. Maybe you sleep badly, feel tense all the time, empty, sad, irritable, or completely overwhelmed. Maybe you do not know whether it is “bad enough”. That is exactly when a family doctor’s practice can be a first step.
You do not have to explain perfectly what is going on. It is enough to say honestly that you are not doing well mentally and need help understanding the situation.
Many people wait too long because they think: “I do not even know what I have.” But that is what a first conversation is for. You do not have to be able to say whether it is depression, anxiety, exhaustion, panic, or something else.
You can simply describe what you notice in daily life:
I hardly sleep anymore.
I can barely get up in the morning.
I am tense all the time.
I withdraw from everyone.
I can hardly manage work, school, training, or household tasks anymore.
I cry a lot or feel nothing at all.
I have fear, panic, or constant pressure in my head.
I notice that I cannot continue like this.
These are not finished diagnoses. They are observations. And observations are enough at the beginning.
If you are afraid of losing your thread during the conversation, you can write down one sentence beforehand. For example:
“I have been feeling mentally unwell for some time. I do not know exactly what is going on, but I need help with an assessment.”
Or even shorter:
“I am not coping well right now and would like to talk about my mental strain.”
Or, if you are very overwhelmed:
“I do not know how to start. Could you please ask me a few questions?”
You may also read the sentence from your phone or bring it on paper.
You do not have to write a long report. Three to five bullet points are enough:
How long have I been feeling this way?
What has changed?
What can I no longer manage right now?
Are there sleep problems, panic, severe exhaustion, or withdrawal?
Are there thoughts of hurting myself?
The last point is especially important. If you have such thoughts, you should say it, even if it is hard. Doctors need this information to assess what help is suitable and safe now.
The family doctor’s practice can be a first place to understand your situation. You can discuss whether physical causes should also be checked, which next steps make sense, and whether further help may be needed.
Possible next steps may include:
another appointment for clarification
a recommendation for psychotherapeutic help
a referral or information about specialist services
information about counseling centers
information about 116117 or the psychotherapeutic consultation hour
in acute danger, immediate emergency assessment
Important: Not every practice can solve everything immediately. But it can be a start so you do not have to sort everything alone.
Some people automatically say in appointments: “It is not that bad.” Even when it actually is bad. This often happens because of shame or habit.
You can therefore write down a more honest sentence beforehand:
“I tend to play it down. Actually, it is affecting me strongly.”
Or:
“I am afraid of not being taken seriously. That is why it is hard for me to speak openly.”
That is also important information.
If even the call is too much, you can prepare a short message if the practice offers email or a contact form:
“Hello, I would like to make an appointment because I have been mentally unwell for some time. Calling is hard for me right now. Could you please tell me an appointment or a possible next step?”
If possible, a trusted person may support you: with wording, with calling, with accompanying you, or with remembering important points.
If you are acutely afraid of hurting yourself, are no longer sure you are safe, or there is immediate danger, do not wait for a normal appointment. In acute danger, call emergency number 112.
